Location: South Salem, NY
Hours: M-F 8am-4pm
Pay Rate: $19.25 / hour.  
Responsibilities: 
  • Track, schedule and accompany people served on medical appointments for adult individuals with developmental disabilities
  • Communicate relevant information to/from the medical providers and the rest of the IDT (interdisciplinary) team
  • Ensure all necessary clinical documentation is prepared and/or obtained as required for each person served in program
  • Ensure proper filing of documentation per agency processes
Requirements:
  • HS or GED equivalent is required; CNA or related college degree/studies preferred
  • Computer competency skills such as Microsoft Outlook, Word, Teams and Excel
  • Valid Driver's license
  • Strong verbal and written communication skills
  • Strong organizational skills  
About the program:
Our group home "Lewisboro" is located in South Salem, NY.  It is a brand new home recently constructed and fully wheelchair accessible with beautiful outdoor scenery. We currently serve 8 individuals. The current population consists of some individuals that are ambulatory, some non-ambulatory and verbal with a diagnosis of TBI or developmental disabilities ranging in age from 28 to 60. Our individuals like to go out in the community to the movies, the mall, the coffee shop, attending concerts, going on walks, working out at the gym, and participating in dinners at other group homes. The majority of individuals require physical assistance with managing their cares, daily activities and community integration. Our amazing and enthusiastic staff teach skills and promote the individuals to do as much as they can for themselves (e.g. not make lunch for individuals but work with individuals to make their own lunches to the best of their abilities).
